Financial Obligations
Your co-pay is due at check in. This is required by your insurance carrier. For your convenience, we accept Mastercard and Visa as well as your personal check or cash.
As a courtesy to you, we bill your primary and secondary insurances. Please remember that even when insurance coverage is available, you are responsible for payment in the event your insurance carrier does not pay. The balance after your insurance carrier has paid is then billed to you. Payment of this balance is expected within 30 days.
Our patient information files need to be updated annually or when any of your information changes. When your insurance carrier requires a referral, please bring it with you at the time of your appointment. We also need to make a copy of your insurance card at each appointment since billing addresses frequently change.
Please be sure to let us know if your insurance carrier or policy changes.
You will be responsible for any non-covered charges at the time of service. An estimate of such charges will usually be discussed with you in advance of the service.
Obstetrical care is billed following delivery. We will assist in obtaining information on your insurance benefits from your insurance carrier. Any balance that is your responsibility should be paid by the end of the seventh month of the pregnancy.
Many insurance carriers require that deliveries and surgeries be pre-certified with the carrier prior to the time of delivery or the procedure. The pre-certification process is usually hospital specific so we need to know your preference as soon as possible. We will usually complete this process for you. If you change the date or the hospital for your procedure, please let us know as soon as possible.
If you do not have insurance coverage, you need to pay at the time services are provided unless other arrangements are made in advance. Cash discounts are available for making prompt payment.
If you have disability insurance coverage and need to have a form completed, or if you need an FMLA form completed, please be sure the employer and employee sections are completed before leaving your form with us. Please allow 5 working days for us to complete the form. There is a nominal charge for this service.
There is a fee of $25 charged when a patient does not keep or cancel two consecutive appointments. We require 24 hours advance notice to cancel an appointment. You may do so by calling 913-384-4990, option 1.
